Trainers use DLL keystroke loggers to listen for when you press particular keys during the game, so it can activate the cheats. DLL-KL.exe's are considered viral by popular AV software because they are used in so many actual viruses, but the nature of trainers ( has to run while the program is open) makes DLL-KL's impossible to dispense with.
